产金属β-内酰胺酶铜绿假单胞菌的耐药性和基因检测

摘    要:目的研究某地区临床分离的铜绿假单胞菌产金属β-内酰胺酶株主要基因型及其耐药性。方法以聚合酶链反应(PCR)法检测该市3所医院2002年8月—2007年6月分离的100株耐亚胺培南和(或)头孢他啶铜绿假单胞菌中产金属β-内酰胺酶基因,并用二倍琼脂稀释法检测10种抗菌药物对产金属酶株的体外最低抑菌浓度(MIC)。结果共检出9株金属酶阳性菌株,其中3株IMP-1扩增阳性;6株VIM扩增阳性,经基因测序确认为VIM-2型。产酶株对头孢哌酮、头孢吡肟、亚胺培南、复方磺胺甲口恶唑全部耐药,部分对阿米卡星、环丙沙星较为敏感。结论该地区铜绿假单胞菌流行株金属酶基因型主要为VIM-2型和IMP-1型;其耐药性强,临床应根据药敏结果选择药物联合使用,以达到最佳抗菌效果。

Abstract:Objective To study the genotypes and antimicrobial resistance of metallo-beta-lactamase-producing Pseudomonas aeruginosa isolated from clinical hospitals in an area. Methods The genes of metallo-beta-lactamase were detected by polymerase chain reaction (PCR) in 100 imipenem and/or ceftazidime-resistant strains of Pseudomonas aeruginosa isolated from 3 hospitals from August, 2002 to June, 2007, and the minimum inhibitory concentration of 10 kinds of antimicrobial agents were determined by agar dilution method in the metallo-beta-lactamase- producing strains. Results Nine strains produced metallo-beta-lactamases, 3 of which were positive in PCR analysis with blaIMP-1 primers, and 6 were blaVIM and identified as VIM-2 by DNA sequence. All the metallo-beta-lacta- mase- producing strains were resistant to cefoperazone, cefepime, imipenem, sulfamethoxazole-trimethoprim, partly resistant to amikacin and ciprofloxacin. Conclusion The main genotypes of metallo-beta-lactamase in epidemic strains of Pseudomonas aeruginosa were VIM-2 and IMP-1, these strains had strong drug-resistance, it is important to choose antimicrobial agents according to antimicrobial susceptibility test.
