| 摘 要： | 目的:探讨双蜡块检测HER-2在结直肠癌组织中表达的临床应用价值，分析HER-2表达与临床病理学特征的相关性。方法:应用全自动免疫组化染色机检测HER-2在228例结直肠癌组织中的表达，用同一张玻片黏附两个不同蜡块的肿瘤组织，均以病理编号小者的蜡块组织表达为第一组结果，两个蜡块组织的联合表达为第二组结果，并对两组数据相应的临床病理学特征进行综合分析。结果:HER-2(0/1+)在第一组中占89.5%(204/228),在第二组中占82.5%(188/228);HER-2(2+/3+)在第一组中占10.5%(24/228),在第二组中占17.5%(40/228)。在第二组中，采用双蜡块联合评分HER-2(2+/3+)的检出率明显高于第一组单蜡块的检出率，两组结果差异具有统计学意义(χ2=4.653,P=0.031)。结直肠癌组织HER-2表达与肿瘤分化程度、浸润深度、淋巴结转移相关(P<0.05),而与性别、年龄、肿瘤直径、发生部位均无相关性(P>0.05)。结论:免疫组化采用双蜡块可以提高HER-2(2+/3+)表达检出率，将提高患者获益于抗HER-2...

| Abstract: | Objective:To study the clinical value of double paraffin blocks for the detection of HER-2 in colorectal cancer,and to analyze the correlation between HER-2 expression and clinicopathological features.Methods:The expression of HER-2 in 228 cases of colorectal cancer was detected by automatic immunohistochemical staining machine.The tumor tissues of two different paraffin blocks were adhered with the same slide.The expression of the paraffin block with the smaller pathological number was the result of the first group,and the joint expression of the two paraffin blocks was the result of the second group.The corresponding clinicopathological features of the two groups were comprehensively analyzed.Results:The percentage of HER-2 score with (0/1+) was 89.5% (204/228) in the first group and 82.5% (188/228) in the second group.The percentage of HER-2 score with (2+/3+) was 10.5% (24/228) in the first group and 17.5% (40/228) in the second group.The detection rate of HER-2 (2+/3+) by double paraffin blocks was significantly higher than that of single paraffin block.The difference between the two groups was statistically significant (χ2=4.653,P=0.031).The expression of HER-2 in colorectal cancer was correlated with the degree of tumor differentiation,depth of invasion and lymph node metastasis (P＜0.05).However,HER-2 expression was not correlated with gender,age,tumor diameter and tumor location (P＞0.05).Conclusion:Immunohistochemistry using double paraffin blocks can improve the detection rate of HER-2 (2+/3+) expression,which will increase the possibility of patients to benefit from anti-HER-2 targeted drug therapy.Over-expression of HER-2 is closely related to the degree of tumor differentiation,depth of invasion and lymph node metastasis.It is suggested that it may be involved in the progression and metastasis of colorectal cancer and help to judge the prognosis. |
