摘    要:目的:比较声脉冲弹性成像(acoustic radiation force impulse, ARFI)中声触诊定量技术(virtual touch quantification, VTQ)、声触诊组织成像定量技术(virtual touch tissue imaging quantification, VTIQ)与超声造影(contrast enhanced ultrasound, CEUS)对甲状腺影像报告和数据系统(TI-RADS)4级结节良恶性的鉴别诊断价值。方法:选取2018年08月至2020年01月在本院接受超声检查的甲状腺结节患者97例(109个结节),根据术后病理结果分为良性组(57例,61个结节)、恶性组(40例,48个结节)。比较两组VTQ、VTIQ检查的剪切波速度(SWV)值及CEUS结果;采用受试者工作特征曲线(ROC)比较VTQ、VTIQ、CEUS对甲状腺TI-RADS 4级结节良恶性的鉴别诊断价值。结果:恶性组VTQ检查的SWV值为[(3.19±0.48) m/s],高于良性组[(2.62±0.46) m/s],且恶性组VTIQ检查的SWV值[(3.81±0...

Abstract:Objective:To compared the acoustic radiation force impulse(ARFI) with virtual touch quantification(VTQ),virtual touch tissue imaging quantification(VTIQ) and contrast enhanced ultrasound(CEUS) in the differential diagnosis value of thyroid TI-RADS grade 4 nodules.Methods:Ninety-seven patients with thyroid nodules(109 nodules) who underwent ultrasound examination in our hospital from August 2018 to January 2020 were selected and divided into benign groups(57 cases,61 nodules) and malignant group(40 cases,48 nodules)according to postoperative pathological results.The shear wave velocity(SWV) values and CEUS results of the two groups of VTQ and VTIQ examinations were compared.Receiver operating characteristic curve(ROC) was used to compare the value of VTQ,VTIQ,and CEUS in distinguishing benign and malignant thyroid TI-RADS grade 4 nodules.Results:The SWV value of VTQ examination in the malignant group was[(3.19±0.48) m/s],which was higher than that of the benign group[(2.62±0.46) m/s],and the SWV value of VTIQ examination in the malignant group was [(3.81±0.52) m/s ],whcih was higher than the benign group[(2.97±0.54)m/s].The difference was statistically significant(P<0.05).The difference between the benign group and the malignant group of CEUS entry time,enhancement form,ring enhancement,regression time,and uniform enhancement was statistically significant(P<0.05).ROC curve showed:AUC of VTQ,VTIQ and CEUS differentiate TI-RADS grade 4 benign and malignant nodules with AUC of 0.815(95%CI=0.734~0.897),0.881(95%CI=0.814~0.949),0.824(95%CI=0.741~0.908).VTQ took 2.975 m/s as the best critical value,and the sensitivity and specificity were 72.92% and 80.33%.VTIQ was 3.615 m/s as the best critical value,and the sensitivity and specificity were 87.50% and 81.97%.CEUS was sensitive.The degree and specificity of CEUS were 81.25% and 83.61%.The sensitivity was highest in VTIQ and CEUS,and the specificity is equivalent.Conclusion:VTQ,VTIQ and CEUS in ARFI have a certain degree of accuracy in the differential diagnosis of benign and malignant thyroid TI-RADS grade 4 nodules.However,VTIQ and CEUS are highly sensitive and have better overall identification efficiency.
