妊娠期糖尿病营养干预的效果及对新生儿预后的影响

摘    要:目的 通过对妊娠期糖尿病患者采用不同的护理方法,探讨妊娠合并糖尿病患者围生期护理及营养干预的效果及对新生儿预后的影响,为临床提供参考。 方法 选择2013年6月—2015年6月期间本院诊治的96例妊娠合并糖尿病患者为研究对象,根据护理方法不同将患者分入对照组与观察组。对照组患者接受常规护理,严格执行各项基础护理,加强饮食护理、运动疗法及新生儿护理。给予观察组患者加强心理护理、健康教育等围生期综合护理措施及营养干预(记录患者年龄、体重、孕周,对患者近3 d的饮食进行研究并计算营养成分的摄入,制订营养计划,告知患者每日摄入总能量,确保孕产妇饮食中三大产能营养素合理配置并有足量的维生素)。比较2组患者空腹血糖(FPG)、餐后2 h血糖、糖化血红蛋白(HbA1c)、甘油三脂(TG)、胆固醇(TC)、低密度脂蛋白(LDL-C)和高密度脂蛋白(HDL-C)。记录妊娠期高血压发生率、剖宫产率及新生儿并发症发生率做为评估母婴预后的指标。 结果 干预后1周观察组患者FPG、餐后2 h血糖显著优于对照组,差异具有统计学意义(P<0.05);出院时观察组患者TG、TC显著低于对照组(P<0.05);与对照组相比,观察组妊娠期高血压发生率、剖宫产率及新生儿并发症发生率显著降低,差异具有统计学意义(P<0.05)。 结论 加强妊娠合并糖尿病患者围生期护理及营养干预可更好地控制血糖和血脂,改善母婴预后。

Effect of nutrition intervention on gestational diabetes mellitus and its effect on the prognosis of neonate

Abstract:Objective To evaluate the effect of perinatal nursing care and nutrition intervention on diabetes during pregnancy and its influence on the outcome of the newborn,and provide reference for clinical treatment. Methods Total 96 cases of diabetes during pregnancy in our hospital from June,2013 to June,2015 were selected and randomly divided into control group and experience group according to the different nursing methods.The control group was given routine nursing care,while the experience group received perinatal nursing care and nutrition intervention.The fasting plasma glucose(FPG),2 h postprandial plasma glucose,glycosylated hemoglobin(HbA1c),triglyceride(TG),cholesterol(TC),low density lipoprotein(LDL-C) and high density lipoprotein(HDL-C) of the two groups were tested.The incidence of gestational hypertension,the rate of cesarean section and neonatal complications were recorded to assess the maternal infant prognosis. Results FPG and PPG on the first week after the intervention in the experience group were superior to the control group(P<0.05);TG and TC on discharge in the experience group were much lower than those in the control group(P<0.05);when compare with the control group,the occurrence of hypertension of pregnancy,rate of cesarean section and the occurrence of neonatal complications in the experience group decreased greatly(P<0.05). Conclusion Strengthening perinatal nursing care and nutrition intervention are benefit for controlling blood sugar and blood lipid and improving the outcome of mother and infants.
