OCTA对飞秒激光辅助白内障超声乳化术后视盘区血管密度和神经纤维层厚度的观察

摘    要:目的 应用光学相干断层扫描血管成像(optical coherence tomography angiography,OCTA)观察飞秒激光辅助白内障超声乳化术后患者视盘区放射状毛细血管密度及神经纤维层厚度的改变。方法 回顾性对照研究。在术前及术后1 d、1个月、3个月,对26眼飞秒激光辅助白内障超声乳化术患者应用OCTA进行视盘区自动化扫描,以获得视盘区放射状毛细血管密度及神经纤维层厚度,同时测量患者最佳矫正视力及眼压。结果 与术前相比,术后1 d、1个月、3个月,飞秒激光辅助白内障超声乳化术患者视盘区放射状毛细血管密度明显减少(F=9.809,P<0.001);术后1 d、1个月、3个月,视盘上半区和下半区的毛细血管密度较术前也均有所减少(F=7.068,P=0.001;F=6.845,P<0.001)。术后1 d,视盘区神经纤维层厚度较术前并无明显变化(P=0.507),但在术后1个月、3个月,视盘区神经纤维层厚度均较术前有所增加(P=0.001、0.019)。术后1 d、1个月、3个月,最佳矫正视力较术前明显提高,差异有统计学意义(F=74.779,P<0.001)。结论 飞秒激光辅助白内障超声乳化术后患者视盘区放射状毛细血管的密度有所减少,视盘区神经纤维层厚度呈晚期增加的趋势,但对患者术后视力的改善并无明显影响。

Clinical observation of optic disc after femtosecond laser-assisted cataract phacoemulsification surgery on OCT angiography

Abstract:Objective To evaluate the changes of radial peripapillary capillary (RPC) density and RNFL thickness after femtosecond laser-assisted cataract surgery (FLACS) using optical coherence tomography angiography (OCTA).Methods Retrospective controlled study.A total of 26 eyes of 26 patients were enrolled in this study.Automated peripapillary maps and capillary density calculations were retrospectively evaluated in 26 eyes using OCTA before and after surgery at 1 day,1 month and 3 months.The best corrected visual acuity (BCVA) and intraocular pressure (IOP) were also measured.Results Retrospective controlled study. A total of 26 eyes of 26 patients were enrolled in this study. Automated peripapillary maps and capillary density calculations were retrospectively evaluated in 26 eyes using OCTA before and after surgery at 1 day, 1 month and 3 months. The best corrected visual acuity (BCVA) and intraocular pressure (IOP) were also measured.Compared with preoperative values, OCTA images revealed a significant decrease in the radial peripapillary capillary (RPC) density at 1 day, 1 month and 3 months postoperatively after FLACS (F=9.809, P<0 001). Furthermore, there were also significant decreases in the superior-hemi and inferior-hemi regions of RPC density (F=7.068, P=0.001; F=6.845, P<0.001). No significant differences were found in peripapillary retinal nerve fibre layer (RNFL) thickness at 1 day postoperatively compared with that of pre-operation (P=0.507). However, the RNFL thickness presented higher at 1 month and 3 months postoperatively (P=0.001, 0.019). There was a significant improvement in BCVA after surgery (F=74.779, P<0.001).Conclusion After femtosecond laser-assisted cataract surgery, the RPC density has an obvious decrease. There was a tendency of RNFL thickening at the late stage of post-operation. However, there was no significant effect on postoperative visual improvement.
