| 摘 要： | 目的 探讨关节镜下射频消融系统治疗膝关节创伤性滑膜炎的临床效果。 方法 选择2013年8月-2015年6月在湖北医药学院附属东风医院诊治的膝关节创伤性滑膜炎患者144例,按随机数字表法分为观察组与对照组各72例,对照组采用关节镜下手术治疗,观察组采用关节镜下射频消融系统治疗,观察2组预后情况。 结果 观察组的术后康复时间与关节积液发生率分别为(9.24±1.24) d和11.1%,都明显低于对照组的(13.24±1.55) d和33.3%(P<0.05)。观察组与对照组术后1个月的膝关节Lysholm评分分别为(86.78±4.29)分和(80.13±5.29)分,都明显高于术前1 d的(41.49±5.33)分和(40.98±4.89)分(P<0.05),同时观察组术后1个月的膝关节Lysholm评分明显高于对照组(P<0.05)。2组术后1个月的患膝关节活动度都明显高于术前1 d(P<0.05),同时观察组术后1个月的患膝关节活动度也明显高于对照组(P<0.05)。观察组术后1个月的切口感染、疼痛等诸多并发症发生情况比对照组明显要少(P<0.05)。 结论 关节镜下射频消融系统治疗膝关节创伤性滑膜炎具有很好的微创性与安全性,能有效改善膝关节功能与关节活动度,从而促进患者的康复。

| Abstract: | Objective To investigate the clinical effects of arthroscopic radiofrequency ablation system in the treatment of post-traumatic synovitis of the knee joint. Methods Total 144 cases of post-traumatic synovitis of the knee joint in our hospital from August,2013 to June,2015 were enrolled and randomly divided into observation group and control group with 72 cases in each group.The control group received arthroscopic surgical treatment,while the observation group was given the arthroscopic radiofrequency.The outcome between the two groups were observed and compared. Results The postoperative recovery time and the incidence of joint effusion of the observation group were (9.24±1.24) d and 11.1% respectively that were significantly lower than (13.24±1.55) d and 33.3% of the control group (P<0.05).The postoperative 1 month Lysholm knee scores in the observation group and control group were (86.78±4.29) points and (80.13±5.29) points,respectively,that were significantly higher than the preoperative 1 d of (41.49±5.33) points and (40.98±4.89) points (P<0.05),while the postoperative 1 month Lysholm knee scores in the observation group were significantly higher than the control group (P<0.05).The postoperative 1 month knee joint activity in the two groups were significantly higher than the preoperative 1 d (P<0.05),while postoperative 1 month knee joint activity in the observation group were also significantly higher than the control group (P<0.05).The postoperative 1 month wound infection,edema,pain,skin pigmentation and other complications in the observation group were significantly lower than those in the control group (P<0.05). Conclusion Arthroscopic radiofrequency ablation system for post-traumatic synovitis of the knee joint has minimally invasive and good security,it can effectively improve knee function and knee joint activity,so as to promote the rehabilitation of patients. |
