贵州省侗族、苗族、布依族成人体质指数与血脂异常的剂量-反应关系

摘    要:目的 探讨贵州省侗族、苗族、布依族体质指数(body mass index,BMI)和血脂异常的剂量-反应关系,为该人群血脂异常防控提供理论依据。方法 基于“中国多民族队列研究”,采用多阶段、分层整群抽样方法,于2018年7月—2019年8月在贵州省黔东南苗族侗族自治州和黔南布依族苗族自治州对16 053名30~79岁侗族、苗族、布依族常住居民进行基线调查,采用多因素logistic回归分析BMI与血脂异常之间的关联,应用限制性立方样条模型分析两者的剂量-反应关系。结果 在总人群中,偏瘦与血脂异常呈负相关(OR = 0.46, 95%CI:0.33 ~ 0.64),超重(OR = 1.55, 95%CI:1.40 ~ 1.72)与肥胖(OR = 1.37, 95%CI:1.16 ~ 1.62)与血脂异常呈正相关;在民族亚组分析中,除了在侗族人群中未发现肥胖与血脂异常关联外,上述关联在三个少数民族中仍然存在。总人群和民族亚组分析均显示BMI与血脂异常风险之间呈倒“U”型非线性剂量-反应关系(Pnon-linear<0.001)。结论 贵州省侗族、苗族、布依族成人BMI与血脂异常风险呈倒“U”型非线性剂量-反应关系。

Abstract:Objective To explore the dose-response relationship between body mass index (BMI) and dyslipidemia in the Dong, Miao, and Bouyei ethnic groups in Guizhou Province, and to provide a theoretical basis for the prevention and control of dyslipidemia. Methods Based on the China Multi-Ethnic Cohort Study, using a multistage, stratified cluster sampling method, 16 053 adults aged 30 to 79 years were selected for a baseline survey in Qiandongnan Miao and Dong Autonomous Prefecture and Qiannan Bouyei and Miao Autonomous Prefecture from July 2018 to August 2019 in Guizhou Province. Multivariate logistic regression was used to analyze the association between BMI and dyslipidemia, and the restricted cubic spline model was used to analyze the dose-response relationship. Results Leanness was negatively correlated with dyslipidemia (OR=0.46, 95%CI: 0.33-0.64), and overweight (OR=1.55, 95%CI: 1.40-1.72) and obesity (OR=1.37, 95%CI: 1.16-1.62) were positively correlated with dyslipidemia. In the ethnic subgroup analysis, except that no association was found between obesity and dyslipidemia in the Dong group, this association still existed in the three ethnic groups. Analysis of the total population and ethnic subgroups showed that there was an inverted U-shaped non-linear dose-response relationship between BMI and the risk of dyslipidemia (Pnon-linear<0.001). Conclusion There is an inverted U-shaped non-linear dose-response association between BMI and dyslipidemia in the Dong, Miao, and Bouyei adults in Guizhou Province.
