| 摘 要： | 目的:探讨单操作孔胸腔镜肺叶切除术治疗早期肺癌的临床效果。方法:回顾性分析2011年1月至2013年12月我院应用VATS治疗的97例早期肺癌患者的病例资料，其中单操作孔VATS手术患者43例（单操作孔组），两操作孔VATS手术患者54例（两操作孔组）。比较手术时间、术中出血量、清扫淋巴结数、胸腔引流管时间及术后住院时间等参数。结果:两组患者均无围手术期死亡。单操作孔组术后引流管留置时间、术后住院时间、术后24小时疼痛评分等均低于两操作孔组（P<0.05）。而术中出血量、手术时间、清扫淋巴结数及术后并发症发生率无显著性差异（P>0.05）。结论:单操作孔VATS肺叶切除并淋巴结清扫术治疗早期肺癌是安全可行的，较两操作孔VATS可减少患者创伤。

| Abstract: | Objective:To evaluate the clinical efficacy of single utility port video-assisted thoracoscopic surgery lobectomy for patients with early-stage lung cancer.Methods:A retrospective analysis was performed on 97 patients with early-stage lung cancer and 43 cases underwent single utility port VATS lobectomy (S-VATS group),the other 54 patients underwent two utility ports VATS lobectomy in the same period（D-VATS group）.The clinical outcomes including operation time，blood loss during the operation，number of dissected lymph nodes，chest drainage duration as well as hospital stay were compared between the two groups.Results:No perioperative death was observed in both groups.There was no surgery-related death.The chest drainage duration，hospital stay and postoperative pain score of the patients in S-VATS group were significantly lower than that of D-VATS group (P<0.05).However，there was no significant difference with regard to blood loss，operation time，number of dissected lymph nodes or postoperative complications (P>0.05).Conclusion:Single utility port VATS lobectomy and lymph node dissection is feasible and safe for patients with early-stage lung cancer which could decrease the operation-related injury. |
