| 摘 要： | 目的:分析三维适形放疗（3D-CRT）联合多西他赛治疗中晚期食管癌的疗效与安全性。方法:将本院2014年1月至2014年12月收治的46例中晚期食管癌患者作为研究对象，根据治疗方式的不同，分为A组（21例）给予3D-CRT治疗，B组（25例）在A组治疗基础上给予多西他赛联合治疗。比较两组患者近期临床疗效、不良反应发生率和3年生存率的差异。结果:B组治疗总有效率为88.00%，较A组的57.14%显著升高（P＜0.05）。两组患者在放射性肺炎、放射性食管炎、血小板减少、血红蛋白减少、白细胞减少等发生率的比较，均无明显差异（均P＞0.05）。虽B组恶心呕吐发生率稍高于A组，但差异无统计学意义（P＞0.05）。B组治疗后3年生存率较A组显著升高（P＜0.05）。结论:3D-CRT联合多西他赛对中晚期食管癌患者的治疗具有良好的临床疗效，且不会引起严重不良反应，患者耐受性良好，具有良好的临床应用价值。

| Abstract: | Objective:To analyze the efficacy and safety of three-dimensional conformal radiotherapy(3D-CRT) combined with docetaxel in the treatment of middle and advanced esophageal cancer.Methods:46 cases of middle and advanced esophageal cancer admitted to our hospital from January 2014 to December 2014 were selected as the research subjects.According to the different treatment methods,46 cases were divided into group A(21 cases) receiving 3D-CRT treatment and group B(25 cases) receiving docetaxel on the basic treatment of group A.The recent clinical efficacy and the incidence of adverse reactions and the differences in the 3 years survival rates were compared between the two groups.Results:The total effective rate of treatment in group B was 88.00%,which was significantly higher than that in group A of 57.14%(P＜0.05).There was no significant difference in the incidence of radiation pneumonitis,radiation esophagitis,thrombocytopenia,hemoglobin reduction and leukocyte reduction between the two groups(all P＞0.05).Although the incidence of nausea and vomiting in group B was slightly higher than that in the control group,but it showed no statistical significance(P＞0.05).The 3-year survival rate in group B was significantly higher than that in group A(P＜0.05).Conclusion:3D-CRT combined with docetaxel has good clinical effect in the treatment of patients with middle and advanced esophageal cancer,and it will not cause serious adverse reactions and patients are well tolerated,so it has good clinical application value. |
