摘    要:目的 探讨血清中细胞因子在矽肺发生发展过程中的变化情况。方法 选择山东某金矿矽肺Ⅰ、Ⅱ期患者、接尘但无矽肺临床表现的工人各40名分别为矽肺Ⅰ、Ⅱ期组、接尘组,另选40名与其他各组均衡可比的地面后勤人员作为对照组,取入选对象血清,应用酶联免疫吸附法(ELISA)测定血清中结缔组织生长因子(Connective Tissue Growth Factor,CTGF)、Ⅱ型肺泡细胞表面抗原(type Ⅱ alveolar cell surface antigen,KL - 6)、白细胞介素 - 17(Interleukin,IL - 17)、单核细胞趋化因子(Monocyte Chemotactic factor - 1,MCP - 1)、基质金属蛋白酶 - 9(Metalloproteinases - 9,MMP - 9)和血小板衍生因子(Platelet Derived Growth Factor,PDGF)的含量。结果 与对照组比,接尘组、矽肺Ⅰ期组和矽肺Ⅱ期组血清中CTGF、KL - 6、PDGF含量均高于对照组(P<0.05),矽肺Ⅰ期组和矽肺Ⅱ期组血清中IL - 17、MCP - 1及MMP - 9含量也高于对照组(P<0.05),且矽肺Ⅰ期组血清中IL - 17、MCP - 1及MMP - 9达到最高值(P<0.05);逐步判别法形成Fisher判别函数,该函数的符合率为92.5%(148/160),误判率为7.5%(12/160);该判别函数对对照组、接尘组、矽肺Ⅰ期组及矽肺Ⅱ期组的正确判别符合率均高于90%。结论 CTGF、KL - 6、IL - 17、MCP - 1、MMP - 9和PDGF做为矽肺早期监测的生物标志物具有重要意义,通过建立多指标联合的判别函数可筛选出用于矽肺病情分级诊断的细胞因子。

Abstract:Objective This work was to investigate the changes in the level of cytokines in the serum of patients with silicosis. Methods A total of 160 workers from one of the Shandong Province gold mines were chosen as the objects of observation. Among them, 40 patients were in the silicosis stage Ⅰgroup, 40 were in the silicosis stage Ⅱ group, 40 who exposed to silica dust without suffering from silicosis Clinical symptoms were in dust-exposed group, and 40 logistical workers without exposure history and without silicosis in the control group. Their basic information was collected by questionnaire. Fasting blood samples were taken from these subjects for collecting the serum. ELISA was used to access the level of connective tissue growth factor (CTGF), type Ⅱ alveolar cell surface antigen (KL-6), interleukin (IL-17), monocytechemotacticfactor-1 (MCP-1), metalloproteinases-9 (MMP-9) and platelet derived growth factor (PDGF) in the serum from subjects. Results In comparison to the control group, the level of CTGF, KL-6 and PDGF in the silicosis stage Ⅰgroup, silicosis stage Ⅱ group and group of dust exposed was higher (P<0.05). The level of IL-17, MCP-1 and MMP-9 in the silicosis stage Ⅰgroup, silicosis stage Ⅱ group was higer than that in the control group (P<0.05) and the level of these reached a peak at the silicosis stage Ⅱ group. Fisher discriminant function was formed by stepwise discriminant, which was in line with 92.5% (148/160), with an error rate of 7.5% (12/160). The discriminant function of the control group, dust-exposed group, silicosis stage Ⅰ group, Ⅱ silicosis group made 90% correct classification. Conclusion As silicosis biomarkers, CTGF, KL-6, IL-17, MCP-1, MMP-9 and PDGF have certain significance, futher suggesting that the rate of early detection of silicosis can be improved by employing the multiple indexes discriminate equation.
