| 摘 要： | 目的 探讨沈阳地区健康体检人群幽门螺杆菌(Hp)感染与甲状腺疾病的相关性,为甲状腺相关疾病的诊疗提供参考依据。方法 将2014年7月至2015年7月在该院健康管理中心进行健康体检的VIP人群作为研究对象,采用C14呼气试验测定幽门螺杆菌感染数目,静脉采血测定幽门螺杆菌抗体和甲功五项指标,甲状腺彩色多普勒超声检查测定甲状腺结节数目和大小。根据幽门螺杆菌感染情况进行分组,分析研究对象中幽门螺杆菌的年龄性别分布特点及其与以上指标的相关性。结果 单因素分析结果显示,HP感染阳性组和阴性组之间在FT3、FT4、TSH水平和甲状腺结节大小、结节是否多发方面的差异无统计学意义,但是TPOAb和TGAb两个抗体滴度方面的差异有统计学意义。多因素Logistic回归分析结果显示,甲状腺自身免疫性抗体是Hp感染的独立影响因素。结论 HP感染与自身免疫性甲状腺炎存在相关性。

| Abstract: | Objective To explore the relationship between the Helicobacter pylori infections and the thyroid disease in a healthy population during a regular checkup in Shenyang City and provide a scientific basis for prevention and treatment of the diseases. Methods The VIP population from the Health Medical Center of the first affiliated hospital of China Medical University from July 2014 to July 2015 were enrolled as the subjects of study.14C-urea breath test was used to detect Helicobacter pylori(Hp)infection.Venous blood was collected to detect Hp-antibodies and thyroid function index.We used Color Doppler ultrasound to test the numbers and sizes of thyroid nodules.Subjects were grouped according to the Hp infection.The distribution characteristics of Hp of the subjects and its relationship with the above indices were analyzed. Results Univariate analysis showed that there were no statistically significant differences in FT3,FT4,TSH levels and size of a thyroid nodule,whether multiple aspects of HP infection between positive and negative groups,but in the two aspects of antibody titers of TPO-Ab and TG Ab,the difference was statistically significant.Multivariate Logistic regression analysis showed that thyroid autoimmune antibodies were independent factors of Hp infection. Conclusion The Hp infection is associated with the autoimmune thyroiditis. |
