| Abstract: | Objective: To evaluate MRI( T2WI + DWI) staging accuracy for endometrial carcinoma( EC) based on the FIGO2009 staging system. Methods: The complete clinical and surgical pathology data from 138 EC patients treated between September 2012 to November 2017 were collected. All patients underwent unenhanced contrast MRI and diffusion-weighted imaging( DWI) procedures. The sensitivity,specificity,accuracy,positive predictive value( PPV) and negative predictive value( NPV) of MR staging were calculated according to histopathology. Results: 138 EC patients were staged according to the FIGO2009 staging criteria including: stage Ⅰ( n = 94),stage Ⅱ( n = 20),stageⅢ( n = 16) and stage Ⅳ( n = 8). The MRI staging accuracy for EC stages Ⅰ-Ⅳ by FIGO2009 were 75. 4 %,8 9. 9 %,9 2. 8 % and 9 8. 6 %,respectively. There was no match between MR staging and histopathology in 30 patients,including 17 cases with under-staging and 13 cases with over-staging. The main causes of misjudgment of the depth of myometrium invasion were blurring and disappearing of junction zone,EC complicated with leiomyoma and cornual lesion. In the diagnosis of cervical stromal invasion,the NPV of MR was higher( 96. 4%). In the diagnosis of lymph node metastasis,MR had high specificity( 99. 2%). Conclusion: According to FIGO2009 staging system,MR T2 WI combined with DWI has high accuracy in EC staging. |
