河北省手足口病伴脱甲症的流行特征分析

摘    要:目的 了解河北省手足口病伴脱甲症的流行及病原学特征,为手足口病伴脱甲症的监测和预防控制提供科学依据。方法 随机抽取河北省5个监测点的实验室诊断病例进行脱甲症回顾性调查。采用荧光定量聚合酶链反应(PCR)对手足口病伴脱甲症病例标本进行复核,同时将标本接种RD细胞进行病毒分离,对其他肠道病毒阳性的标本进行柯萨奇病毒A组6型(Coxsachievirus A6,Cox A6)和柯萨奇病毒A组10型(Coxsachievirus A10,Cox A10)型别鉴定。结果 1741例手足口病患者中45例出现指(趾)甲脱落症状,脱甲率为2.6%,男女比例为1.4:1,平均年龄(1.81.3)岁,1~岁病例数占51.1%。脱甲发生在发病前10天到发病后86天不等,平均(29.616.4)d。指(趾)甲脱落程度不等,指甲与趾甲的脱落率分别为82.2%和42.2%,指甲脱落率高于趾甲脱落率(2=15.3,P0.01)。45例手足口病伴脱甲症患者中其他肠道病毒感染43例,其中41例为Cox A6,2例为埃可病毒30(Echovirus 30,ECHO30)。结论 河北省引起手足口病伴脱甲症的主要病原是Cox A6;脱甲症主要发生于手足口病后20~40天,指甲脱落率高于趾甲脱落率。

Epidemiology of hand foot and mouth disease related onychomadesis in Hebei

Abstract:Objective To understand the epidemiologic and etiologic characteristics of onychomadesis related with hand foot and mouth disease(HFMD) in the surveillance areas in Hebei province and provide scientific evidence for surveillance, prevention and control of HFMD onychomadesis. Methods A retrospective epidemiologic survey was conducted among the laboratory confirmed HFMD cases randomly selected in 5 surveillance areas in Hebei. The specimens of the HFMD cases accompanying with onychomadesis were re-detected with real time PCR and Cox A6 or Cox A10 identifications were conducted for the positive specimens of other enteroviruses. Results Onychomadesis occurred in 45 of the 1741 HFMD cases(2.6%). The male to female ratio of the cases was 1.4:1. The average age of the cases was 1.81.3 years(ranging from 0 to 6 years old). The cases in age group 1 year accounted for 51.1%. Onychomadesis occurred in the cases from 10 days before HFMD onset to 86 days after HFMD onset. The fingernail loss rate was 82.2% and the toenail loss rate was 42.2%(2=15.3, P0.01). Among the 45 HFMD related onychomadesis. 43 were infected with other enterovirusses, i.e. 41 Cox A6 infections and 2 ECHO30 infections. Conclusion Cox A6 was the major pathogen causing HFMD related onychomadesis in Hebei. Onychomadesis mainly occurred at 20 to 40 days after HFMD onset. The rate of finger-nail loss was higher than that of toenail loss.
