乙酰半胱氨酸对角膜新生血管形成过程中瘦素和PEDF表达的调节

摘    要:目的 观察大鼠碱烧伤后不同时期角膜组织病理学变化、瘦素和色素上皮衍生因子(pigmentepitheliumderivedfactor,PEDF)的表达以及乙酰半胱氨酸对瘦素和PEDF表达的影响,探讨他们与角膜新生血管(cornealneovascularization,CNV)的关系。方法 使用碱烧伤诱导大鼠CNV模型,采用浸润1mol?L-1NaOH溶液,直径为3.0mm的单片滤纸,准确置于大鼠右眼角膜中央23s制作CNV模型。75只大鼠随机分成3组,每组25只,左眼为正常对照眼。A组自烧伤之日起每天滴注PBS液;B组每天滴注乙酰半胱氨酸;C组烧伤后7d之内每天滴注PBS液,7d后每天加滴乙酰半胱氨酸。用裂隙灯显微镜观察CNV,分别计算CNV长度和面积,并行HE染色和免疫组织化学检测。结果 B组CNV生长趋势与A组、C组基本相似,但较A组、C组生长迟缓,C组比A组缓慢,但稍快于B组。C组角膜病理变化与A组相似,较B组稍重。除4d时A组和C组间CNV长度及面积差异均无统计学意义(均为P>0.05)外,余时间点组间两两比较差异均有统计学意义(均为P<0.05)。免疫组织化学检测显示角膜碱烧伤后,瘦素和PEDF的表达开始增高。伤后7d,各组瘦素表达:A组、B组、C组分别为0.2761±01144、02322±0.0749、0.2692±0.0646,PEDF表达分别为0.3521±0.1074、0.3899±0.0721、0.3561±0.1233,三组瘦素、PEDF表达总体比较差异有统计学意义(P<0.05);两两比较显示,A组与B组、B组与C组瘦素、PEDF表达差异均有统计学意义(均为P<0.05)。伤后14d,各组瘦素与PEDF的表达总体比较差异均有统计学意义(均为P<0.05);各组间两两比较显示,瘦素、PEDF表达差异均有统计学意义(均为P<0.05)。结论 瘦素和PEDF表达水平与CNV的生成具有相关性,乙酰半胱氨酸能有效降低瘦素的表达,提高PEDF的表达。

Abstract:Objective To observe the histopathological changes,leptin and pigment epithelium derived factor ( PEDF) expression. as well as effects of acetylcysteine on leptin and PEDF expression in different periods after corneal alkali burns. and explore their relationship with corneal neovascularization ( CNV) . Methods CNV models were established by putting the filter paper with the diameter of 3 mm immersed into I mol . L -l NAOH solution on the central corneas of right eyes of rats for 30 seconds. Right eyes of 75 rats were treated as experimental group and divided into groups A,B and C,25 eyes in each group. Left eyes were treated as normal control group. Starting from the date of the burn , group A were dropped with PBS , group B with acetylcysteine 5 times per day, and group C with PBS within 7 day, then with acetylcysteine 5 times a day from 7 days t0 14 days. CNV was observed by slit lamp microscope every day, and the length and area of CNV were calculated and detected by HE staining and immunohistochemical method. Results The growth tendency of CNV in group B was primary similar to group A and C,the growth rate of group B was lower than those of group A and C ,and group C was lower than group A. The corneal histopathological changes in group C were similar to group A,but more server than group B. Except for 4 days after alkali burn , there were statistical differences in the length and area of CNV at other time points between group A and C ( all P < 0. 05 ) . Immunohistochemical detection showed that,the expression of leptin and PEDF began to rise after corneal alkali burn. The expression of leptin at 7 days after alkali burn in group A,B and C were 0. 273 I +0. 114 4 . 0. 232 2 + 0. 074 9 and 0. 269 2 + 0. 064 6 , respectively, the expression of PEDF were 0. 352 I +0. 107 4 ,0. 389 9 +0. 072 I and 0. 356 I +0. 123 3 ,respectively , there was significant difference among three groups (P < 0. 05 ) . and there were statistical differences between group A and B , group B and C ( all P < 0. 05) . At 14 days after alkali burn , there was significant difference in the expression of leptin and PEDF among three groups(P < 0. 05 ) . and there were statistical differences between each two groups ( all P < 0. 05 ) . Conclusion Expressions of leptin and PEDF are related with CNV. Acetylcysteine can effectively reduce the expression of leptin , enhance the expression of PEDF.
