急进高原卫生应急人员血氧饱和度及心率与高原反应症状发生关系研究

摘    要:目的 了解急进高原地区卫生应急队员急性高原反应(Acute Mountain Sickness,AMS)症状发生情况及其与血氧饱和度、心率相关关系。 方法 在高原开展卫生应急演练期间,对24名卫生应急队员进行了相关指标监测,分析在高原低压低氧环境下的AMS发生情况,及其与血氧饱和度、心率相关关系。 结果 急进海拔3 500 m以上高原地区开展卫生应急工作,50%(12例)的调查对象出现AMS自觉反应中的一种或以上,早晚血氧饱和度( t =0.992, P =0.332)和心率( z =1.09, P =0.274)差异无统计学意义,不同自觉症状组别间血氧饱和度、心率的平均值、差值间的差异无统计学意义( P >0.05)。 结论 血氧饱和度和心率应作为急进高原人员身体状况监测的重要指标,但若依据血氧饱和度、心率等客观指标判别急进高原人员AMS状况时,需结合头晕、头痛、乏力、恶心、呼吸困难、睡眠障碍等AMS典型自觉症状。

Abstract:To understand the situation of acute mountain sickness(AMS) symptoms of emergency worker rapidly ascended to the plateau and the relationship between blood oxygen saturation, heart rate and AMS symptoms. Methods A health emergency exercise was carried out on the plateau, relevant index monitoring was carried out on 24 members, the AMS occurrence situation in the plateau low-pressure low-oxygen environment was analyzed, and the correlation relation with the blood oxygen saturation and the heart rate was achieved. Results Health emergency work on a plateau area with the elevation of more than 3 500 m was carried out, wherein 50%(12 cases) of survey objects appeared one or more of AMS conscious reactions. There was no significant difference between the blood oxygen saturation(t=0.992, P=0.332) and heart rate(z=1.09, P=0.274) difference both in the morning and evening. There was no significant difference in the blood oxygen saturation and the heart rate average value among the different consciousness symptom groups(P>0.05). Conclusion The blood oxygen saturation and the heart rate should be used as an important index for monitoring the physical condition of the emergency plateau personnel, but if the AMS is determined according to the blood oxygen saturation and heart rate, the AMS conscious reactions such as dizziness, headache, weakness, nausea, dyspnea, sleep disorders are required to be combined.
