摘    要:目的 研究新型冠状病毒感染肺炎(COVID - 19)患者外周血淋巴细胞亚群的变化情况,探讨其临床意义,为该类疾病的临床诊断、治疗提供参考。方法 以2020年1月31日 - 3月31日期间武汉大学人民医院东院收治的91例确诊COVID - 19患者作为研究对象,回顾分析其入院后第一次实验室检查的淋巴细胞计数及各类亚群计数,通过方差分析(ANOVA)比较不同疾病严重程度患者之间的差异。结果 随着疾病严重程度的增加,淋巴细胞计数、T淋巴细胞计数(CD3+)、CD4+T淋巴细胞计数、CD8+T淋巴细胞计数均逐渐减低,差异有统计学意义(F = 27.966,P<0.001;F = 53.574,P<0.001;F = 42.563,P<0.001;F = 34.949,P<0.001)。危重型CD4+/CD8+比值呈异常升高状态,高于普通型、重型,差异有统计学意义(F = 4.849,P = 0.010)。危重型B淋巴细胞计数(CD19+)、NK淋巴细胞计数(CD16+CD56+)均低于其他两组,差异有统计学意义(F = 4.048,P = 0.021;F = 3.667,P = 0.030),普通型、重症型之间差异无统计学意义。结论 COVID - 19患者的淋巴细胞亚群计数与疾病的严重程度密切相关,有助于对预后的判断。

Abstract:To study the changes of peripheral blood lymphocyte subsets in COVID-19 patients with different clinical consequences, to explore the clinical significance, and to provide reference for the clinical diagnosis and treatment of this disease.Methods A total of 91 confirmed COVID-19 patients admitted to the East District of Wuhan University People’s Hospital from2020-01-31 to 2020-03-31 were enrolled. The lymphocyte count and subsets counts of the first laboratory examination after admission were retrospectively analyzed. The differences among patients with different disease severity were compared by ANOVA.Results With the increase of disease severity, lymphocyte count, T lymphocyte count(CD3+), CD4+T lymphocyte count and CD8+T lymphocyte count all decreased gradually(F=27.966, P<0.001; F=53.574, P<0.001; F=42.563, P<0.001; F=34.949, P<0.001, respectively).The CD4+/CD8+ ratio in the critical severe group increased abnormally, which was significantly higher than that in the normal group and the severe group, and the difference was statistically significant(F=4.849, P=0.010). B lymphocyte count(CD19+) and NK lymphocyte count(CD16+CD56+) in the critical severe group were lower than those in the other two groups(F=4.048, P=0.021; F=3.667,P=0.030, respectively). There was no statistically significant difference between the normal group and the severe group. Conclusion The lymphocyte subsets counts of COVID-19 could reflect the severity of the disease, which is helpful for the prediction of prognosis.
