| 摘 要： | 目的:研究肠内营养对老年肿瘤化疗患者免疫及营养指标的影响，为临床治疗提供参考。方法:回顾性分析我院于2015年11月至2017年11月收治的老年肿瘤化疗172例作为研究对象，其中对照组86例只行单纯化疗，观察组86例采用肠内营养支持化疗。对比两组患者化疗2周后营养及免疫功能指标变化。结果:化疗2周后，两组患者的IgA、IgG、中性粒细胞的趋化指数、IgM免疫球蛋白水平、吞噬率、黏附率、NK细胞数、杀菌率等免疫学指标均较治疗前显著降低（P<0.05），但观察组优于对照组（P<0.05），差异有统计学意义（P<0.05）。化疗2周后，观察组前白蛋白（prealbumin，PA）、白蛋白（albumin，ALB）、血红蛋白（hemoglobin，Hb）、SOFA评分及APACHEII评分等营养指标均较治疗前明显改善（P<0.05），但对照组与治疗前无差异（P>0.05）。结论:围化疗期肠内营养可有效改善老年肿瘤化疗患者免疫机能，提高其营养学指标，有助于化疗后康复，值得临床借鉴。

| Abstract: | Objective:To study the effect of enteral nutrition on immunological and nutritional indexes in elderly patients with tumor chemotherapy，and to provide reference for clinical treatment.Methods:A retrospective analysis of 172 cases of elderly tumor chemotherapy in our hospital from November 2015 to November 2017 were conducted，including 86 patients in the control group who only received chemotherapy alone，and the observation group was treated with enteral nutrition support chemotherapy.The changes of nutritional and immune function indexes were compared between the two groups after 2 weeks of chemotherapy.Results:After 2 weeks of chemotherapy，two patients of IgA，IgG，neutrophil chemotactic index，IgM immunoglobulin levels，phagocytic rate，adhesion rate，NK cell number，the sterilization rate and immunological indexes were significantly lower than those before treatment(P<0.05)，but the observation group than the control group(P<0.05)，the difference was statistically significant(P<0.05).After 2 weeks of chemotherapy，the nutritional indicators such as prealbumin(PA)，albumin(ALB)，hemoglobin(Hb)，SOFA score and APACHEII score in the observation group were significantly improved compared with those before treatment (P<0.05)，but there was no difference between the control group and that before treatment(P>0.05).Conclusion:Perioperative enteral nutrition can effectively improve the immune function of elderly cancer patients，improve their nutritional indicators，and contribute to the rehabilitation after chemotherapy，which is worthy of clinical reference. |
