| 摘 要： | 患者,男,41岁。躯干、双侧大腿上部散在触痛性结节、斑块54天。发病前有大腿外伤史及河水接触史。皮肤组织病理检查示:真皮深部感染性肉芽肿改变。细菌培养及菌落DNA PCR测序示偶发分枝杆菌。患者行全外显子测序未发现免疫、感染相关突变,综上诊断为偶发分枝杆菌感染性肉芽肿。给予左氧氟沙星、克拉霉素、脓肿切开、穿刺引流等,病情好转。

| Abstract: | A 41-year-old man presented with a 2-month history of scattered tender nodules, plaques in his trunk and lower limbs. Before the onset of lesions, he was injured on the lateral right thigh and swam in a river. Histopathological examination revealed a dermal infectious granuloma. Mycobacterium fortuitum was confirmed by bacteria identification and PCR. No immunological or infection-related mutation was found by whole exome sequencing. The diagnosis of infectious granuloma due to Mycobacterium fortuitum was made. The patient was treated with levofloxacin, clarithromycin, abscess incision, drainage etc., and the condition improved obviously. |
