2015 - 2016年江苏省0~15岁健康儿童肠道病毒携带情况调查

摘    要:目的 了解江苏省2015 - 2016年15岁以下健康儿童肠道病毒(enterovirus,EV)的携带情况及病毒型别的特征分布。方法 采集江苏省3个县(市)15岁以下健康儿童粪便标本445份,进行病毒分离和鉴定。脊灰病毒(PV)采用荧光定量RT - PCR方法进行鉴定;非脊灰肠道病毒(NPEV)采用RT - PCR扩增结合VP1区基因测序定型。结果 共检测到EV31株,阳性率6.97%(31/445)。其中PV18株,阳性率4.04%(18/445),包含Ⅰ型8株、Ⅱ型7株、Ⅲ型3株,未分离到混合型PV。所有的PV毒株经鉴定均为疫苗株,无脊灰野病毒株与疫苗衍生株。NPEV13株,阳性率2.92%(13/445),以HEV - B群(84.62%(11/13))为主。所有NPEV分离株与对应标准株的核苷酸序列同源性在92.01%~97.42%之间,氨基酸序列同源性在92.91%~99.19%之间。构建进化树进行同源性分析发现,所有的NPEV分离株与对应的标准株聚集在一起,符合测序定型标准。结论 在2015 - 2016年度江苏省健康儿童中,PV阳性率与急性迟缓性麻痹(AFP)监测相当,但NPEV阳性率远低于AFP监测。检测到的NPEV以HEV - B群为主,未检测到脊灰野病毒,江苏省继续保持无脊灰状态。

Abstract:Objective To explore the prevalence and distribution of human enterovirus(HEV) infection among healthy children under the year of 15 in Jiangsu province from 2015 to 2016. Methods A total of 445 stool samples were collected from healthy children under the year of 15 in three counties of Jiangsu province. All stool samples were cultured in cells and positive isolates were tested by realtime RT-PCR. The molecular typing and phylogenetic analysis of non-polio enterovirus(NPEV) were based on VP1 region gene sequencing. Results 31 EVs were isolated from 445 stool samples and the positive rate was 6.97%. 18 PVs were isolated and the positive rate was 4.04%. All PVs were detected as vaccine strains. 13 NPEVs (2.92%) were isolated, with 11 strains NPEVs corresponding to HEV-B(84.62%). The sequence of the VP1 region of the all NPEV strains showed 92.01% to 97.42% nucleotide and 92.91% to 99.19% amino acid homology with reference HEV strains. Molecular phylogeny indicated that all NPEV isolates clustered together with the respective reference sequences, supported by high bootstrap values, further confirming the serotype identification. Conclusion The PVs isolating rate in healthy children was similar to that in acute flaccid paralysis(AFP) surveillance patients in Jiangsu from 2015 to 2016. However, the NPEVs isolating rate was lower than AFP surveillance children(2.92% vs. 4.70%). HEV-B was shown as the predominant species from 2015 to 2016. No wild-type PVs were detected and polio-free status was maintained well in Jiangsu.
