| 摘 要： | 目的 探讨颅内动脉瘤开颅夹闭术后动脉瘤残留的原因及其预防.方法 总结我院2005～2012年颅内动脉瘤夹闭术115例,其中113患者在夹闭术后1个月行3D-DSA检查观察动脉瘤夹闭情况,动脉瘤残留的纳入标准为在瘤夹近端仍有部分动脉瘤显影.结果 115例患者术后行3D-DSA检查113例,其中103例完全夹闭,4例出现载瘤血管狭窄（相对于术前）;动脉瘤残留6例（5.31％）,残留病例中2例患者再次行动脉瘤介入栓塞术,余4例仍在随访中,6例患者预后良好.结论 开颅夹闭术后动脉瘤的残留与术前准备、手术技巧、器械及术中影像评估有相当大的关系,通过各个方面的改善及完善,动脉瘤夹闭术后残留是可以预防的. |

| Abstract: | Objective To analyze the reason of residual aneurysms after aneurysm clipping and study how to avoid it.Methods 115 cases with aneurysm clipping were retrospectively analyzed.DSA postoperative examinations were performed in 113 cases after operation.Results 113 cases were clipped completely.There were five cases of parent artery stenosis in these cases.Six residual aneurysms were found by postoperative angiography （5.31％）.Two case of them were underwent interventional embolization therapy,the other four cases are followed up.Conclusion Residual aneurysms after aneurysm clipping can be prevented by preoperative preparation,surgical technique and imaging evaluation. |
